Don't forget to bring a current list of medications, a general practitioner's note of referral, and your insurance card.
Due to high demand for our care, if you need to cancel your appointment remember to do so at least 24 hours before your appointment or you will be billed a 'no show' fee of $50. This is to ensure that we can provide a high level of attention to our patient population.
